Job summary

At Home Healthcare, part of the Optum family, is seeking an Occupational Therapist in Home Health to assess and evaluate patient care needs related to functional status, ADLs, and home adaptations. You will work within the plan of care with the health care team to support patient independence.

The role requires state licensure, CPR certification, reliable transportation, and the ability to work in diverse home environments.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides service within the scope of practice as defined by the state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other health care team members.
• Evaluates the patient’s functional status and body systems as required for CMS documentation; collaborates with physician on therapy plan of care.
• Observes, records, and reports to the nurse supervisor and physician the patient’s response to treatment and changes in condition.
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.
