Job summary

True North Consultants, Inc. in Connecticut seeks an Occupational Therapist to deliver high-quality OT services in clients' homes, conducting assessments, developing treatment plans, and coordinating with physicians and care teams.

The ideal candidate has an accredited OT program, state license, CPR certification, and at least one year of home health experience. You will educate clients and families, select adaptive devices, and ensure compliance with policies and standards.

Qualifications

• Graduate of an accredited occupational therapy program, or eligible for national exam.
• Current OT license/registration as required by state of practice.
• Minimum one year experience in a home health setting or equivalent.
• CPR certification.

Responsibilities

• Performs occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation of clients under a medically approved Plan of Care.
• Develops a treatment program and establishes goals for improved function.
• Communicates plans and changes to physician, nursing Case Manager, and other staff; participates in care conferences.
• Instructs clients and their families in client-specific programs and activities to promote independence.
• Designs and orders appropriate adaptive/assistive devices and splints as needed.
• Attends care conferences, initiates interdisciplinary referrals, and participates in care coordination.
