Qualifications

• Current CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Experience in home health care preferred.
• Experience with high acuity patients a plus.

Responsibilities

• Evaluate and address home-based needs to support independence.
• Coordinate with Case Manager and the care team across patient homes.
• Educate patients and caregivers on safety, pain management, and functional strategies.
• Support interdisciplinary collaboration and care planning within the patient's environment.
