CareerNerds, LLC is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization in Washington. The role operates within the Patient Centered Medical Home model, collaborating with physicians, ARNPs, PAs, and nursing staff to deliver high-quality care across a diverse patient population.

The position requires ARNP program graduation, WA RN/ARNP licensure, WA prescriptive authority, and broad primary care clinic experience.

Formación

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in the State of Washington.
• Prescriptive authority from Washington State.
• Experience practicing in a broad primary care clinic setting.

Responsabilidades

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients.
• Maintain accurate chart documentation within a 48-hour timeline (95% fidelity).
• Meet productivity expectations (minimum 3,400 encounters per year).
• Provide care coordination through daily huddles as part of PCMH model.
• Identify at-risk populations and monitor panel health.
• Empower patients through self-management education and discharge planning.
• Advocate for patients by explaining procedures and plans.
