Job summary

CareerNerds, LLC is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health care team in Washington. This full-time role operates within the Patient Centered Medical Home model and collaborates with physicians, PAs, ARNPs, and nursing staff to deliver high-quality, comprehensive care to a diverse patient population.

Qualifications

• Graduate of an accredited ARNP program
• Current RN and ARNP licensure in the State of Washington
• Prescriptive authority from Washington State
• Experience practicing in a broad primary care clinic setting
• Experience in minor surgical techniques (preferred)
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management
• Develop and implement individualized teaching plans for patients
• Use independent clinical judgment based on evidence-based data
• Maintain timely chart documentation (48-hour timeline, 95% of the time)
• Complete fee slips with matching diagnosis and procedure codes
• Meet productivity expectations (minimum 3,400 encounters per year)
• Provide care coordination through daily huddles in the PCMH model
• Identify at-risk patient populations and monitor panel health
• Educate patients for self-management and discharge planning
• Explain procedures and treatment plans to patients
• Participate in quality management and staff education
• Support clinical education of student nurses
• Maintain annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment)
