LRSolutions, LLC is seeking an experienced Advanced Registered Nurse Practitioner (ARNP) to join a patient-centered, community-based healthcare team operating within the PCMH model. You will collaborate with physicians, PAs, ARNPs, and nursing staff to deliver high-quality care to a diverse patient population.

The role requires ARNP program graduation, WA RN/ARNP licensure, WA prescriptive authority, and solid primary care clinic experience.

Qualificações

• ARNP program graduate required.
• Current RN and ARNP licensure in WA required.
• Prescriptive authority in WA required.
• Experience in a broad primary care clinic setting required.
• Strong knowledge of nursing process: assessment, planning, implementation, and evaluation.

Responsabilidades

• Perform advanced clinical assessments and manage illnesses within ARNP scope of practice.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients in achieving and maintaining optimal health.
• Maintain timely and accurate chart documentation (within 48-hour timeline, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year, full-time equivalent).
• Provide care coordination through daily huddles as part of the PCMH model.
