Professional Recruiting Consultants, Inc. is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization in Washington.

This full-time, permanent position operates within the Patient Centered Medical Home model and collaborates with physicians, ARNPs, PAs, and nursing staff to deliver comprehensive care. The role emphasizes independent clinical judgment, care coordination, and patient education, with a focus on quality

Qualifikationen

• ARNP program completion required.
• Washington state RN/ARNP licensure required.
• Prescriptive authority in Washington required.
• Experience in broad primary care setting required.
• Minor surgical techniques experience preferred.

Aufgaben

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patients to achieve health goals.
• Use independent clinical judgment based on evidence-based data.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Meet productivity expectations (min 3,400 encounters per year, FT).
• Provide care coordination through daily huddles as part of PCMH model.
• Identify at-risk populations and monitor panel health.
• Educate patients for self-management and discharge planning.
• Explain procedures and treatment plans to patients before execution.
• Participate in quality management and peer review; support clinical education of students.
