Client Growth Resources, Inc. seeks an experienced Advanced Registered Nurse Practitioner in Washington to join a patient-centered, community-based healthcare team. You will collaborate within the PCMH model to deliver high-quality care and coordinate services across clinicians.

Requirements include WA RN/ARNP licensure and prescriptive authority; strong primary care background and commitment to patient education. Relocation assistance is available as part of a comprehensive benefits package.

Qualifications

• Graduate of an accredited ARNP program.
• Active RN and ARNP licensure in the State of Washington.
• Prescriptive authority in Washington.
• Experience in broad primary care clinical settings.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nurses on patient care.
• Develop and implement teaching plans to support patient health.
• Maintain chart documentation within a 48-hour timeline (95%+).
• Meet productivity targets (3,400 encounters/year for FT).
• Provide care coordination through daily huddles in PCMH model.
• Identify at-risk populations and monitor practice panel health.
• Educate patients for self-management and discharge planning.
