Talent With Heart is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare team in Washington. This permanent, full-time role operates within the PCMH model and collaborates closely with physicians, ARNPs, PAs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population.

Qualifikationen

• ARNP program graduate is required.
• Current WA RN and ARNP licensure required.
• Prescriptive authority from Washington required.
• Broad primary care clinic experience required.
• Minor surgical techniques experience preferred.
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation.

Aufgaben

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patients.
• Provide sound clinical judgment based on evidence-based data.
• Maintain chart documentation within a 48-hour timeline (95% of the time).
• Complete fee slips with appropriately matching diagnosis and procedure codes.
• Meet productivity expectations (minimum 3,400 encounters per year).
• Provide care coordination through daily huddles in PCMH model.
• Identify at-risk patient populations and monitor panel health.
• Educate patients for self-management and discharge planning.
• Explain procedures and treatment plans prior to execution.
• Participate in quality management and clinical education of staff.
