Job summary

Your Talent Team in Washington State seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health organization operating within the PCMH model.

You will collaborate closely with physicians, PAs, ARNPs and nursing staff to deliver high-quality care across diverse populations, perform advanced assessments, manage illnesses, coordinate care, and support patient education and self-management.

Qualifications

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in Washington.
• Prescriptive authority in Washington.
• Experience practicing in a broad primary care clinic setting.
• Strong knowledge of the nursing process: assessment, planning, implementation, evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs and nursing staff on patient care management.
• Develop and implement teaching plans to support patient health goals.
• Maintain timely chart documentation (within 48-hour timeline, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year, full-time).
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk patient populations and monitor panel health.
