Vogrinc Search Group is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered community healthcare team in Washington state. This full-time position works within a Patient Centered Medical Home model, collaborating with physicians, PAs, ARNPs, and nursing staff to provide comprehensive care across diverse patient populations.

Qualifications

• Graduate of an accredited ARNP program (required).
• Current RN and ARNP licensure in the State of Washington (required).
• Prescriptive authority from Washington State (required).
• Experience practicing in a broad primary care clinic setting (required).
• Experience in minor surgical techniques (preferred).
• Strong knowledge of the nursing process including assessment, planning, implementation and evaluation.

Responsabilités

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care.
• Develop and implement individualized teaching plans for patient health.
• Maintain timely chart documentation within 48 hours (95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year).
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk populations and monitor panel health.
• Discharge planning and patient self-management education.
