Connectly Recruiting in Washington state seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health organization operating in the PCMH model. The role collaborates with physicians, PAs, ARNPs, and nurses to provide high-quality primary care.

Qualifications

• ARNP program graduate (required)
• Current RN and ARNP licensure in Washington (required)
• Prescriptive authority in Washington (required)
• Experience in broad primary care clinic setting (required)
• Experience in minor surgical techniques (preferred)
• Strong nursing process knowledge (assessment/planning/implementation/evaluation)

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management
• Develop and implement individualized teaching plans for patients
• Use independent clinical judgment based on evidence-based data
• Maintain chart documentation within 48-hour window (95% of the time)
• Meet productivity goals (3,400 encounters/year full-time)
• Provide care coordination through daily huddles in PCMH model
• Identify at-risk populations and monitor panel health
• Educate patients for self-management and discharge planning
• Explain procedures and treatment plans to patients prior to execution
• Support clinical education of health professionals including students
• Maintain all annual trainings (HIPAA, Cultural Diversity, BBP, Sexual Harassment)
