Job summary

Vogrinc Search Group is seeking an Advanced Registered Nurse Practitioner in Washington to join a patient-centered, community-based healthcare organization operating in a Patient-Centered Medical Home model.

The ARNP will collaborate with physicians, PAs, and nursing staff to deliver comprehensive primary care, perform advanced assessments, care coordinate, educate patients, and maintain thorough charting within 48 hours while meeting annual encounter targets.

Qualifications

• ARPN program graduate (required)
• Current RN and ARNP licensure in WA (required)
• Prescriptive authority in WA (required)
• Experience in broad primary care clinic setting (required)
• Experience in minor surgical techniques (preferred)
• Strong knowledge of nursing process: assessment, planning, implementation, and evaluation

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management
• Develop and implement individualized teaching plans for patients
• Use independent clinical judgment based on evidence-based data
• Maintain chart documentation within 48 hours (95% of the time)
• Meet productivity targets: 3,400 encounters per year
• Provide care coordination through daily huddles in PCMH model
• Identify at-risk populations and monitor panel health
• Discharge planning and patient self-management education
• Support clinical education of students and staff
• Maintain required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment)
