Job summary

Atrius Health, part of the Optum family, is seeking a Primary Care Provider - Advanced Practice Clinician (NP or PA) to join our Weymouth, MA team. You will independently manage a defined primary care panel in an ambulatory setting, performing comprehensive assessments and coordinating care with an interdisciplinary team.

Qualifications

• NP/PA license with active, unrestricted MA license (or ability to obtain before start)
• National certification (ANCC or AANP) for NP or NCCPA for PA
• Active DEA and MCSR (or ability to obtain before start)
• Current BLS certification
• Minimum of three (3) years of independently managed primary care panel
• Ambulatory primary care experience and experience with inbox management, test results, patient messaging, referrals

Responsibilities

• Independently manage a defined panel of primary care patients
• Perform comprehensive assessments, diagnoses, and treatment in ambulatory primary care
• Manage acute and chronic conditions; author and supervise care plans
• Determine in-office management vs referral to specialty or emergency care
• Handle inbox duties: results, messages, meds, referrals
• Coordinate care with RNs, pharmacists, population health, behavioral health, and specialists
• Support shared decision-making and whole-person care
• Provide consultation on hospitalized patients within the panel
• Support practice efficiency, quality initiatives, and standardized workflows
