Job summary

Atrius Health, part of the Optum family, seeks a Primary Care Provider - Advanced Practice Clinician (NP or PA) to join our Weymouth, MA team. You will manage a defined panel of primary care patients in an ambulatory setting, delivering longitudinal, relationship-based care.

We support clinicians with Optum resources, including care coordination, data analytics, and a focus on value-based care. This role offers growth opportunities, CME support, and a collaborative, team-based environment.

Qualifications

• 3+ years independently managing a full primary care patient panel
• Active MA NP or PA license (or ability to obtain before start)
• Active DEA and MCSR (or ability to obtain before start)
• Current BLS certification

Responsibilities

• Independently manage a defined panel of primary care patients in ambulatory settings
• Perform comprehensive assessments, diagnoses, and treatment of patients
• Manage acute and chronic conditions; author and supervise patient care plans
• Coordinate care with RNs, pharmacists, population health, behavioral health, and specialists
• Handle inbox tasks: test results, patient messaging, medication management, referrals
• Support practice efficiency and standardized clinical workflows
• Provide consultation regarding hospitalized patients within the assigned panel
