Job summary

Atrius Health, part of the Optum family, seeks an Advanced Practice Clinician (NP or PA) to join our Boston care team. You will practice in a collaborative, value-based environment with an independent primary care panel and robust care coordination.

Qualified APCs have Massachusetts NP/PA licensure, MSN or NCCPA, and 3+ years in ambulatory primary care. Epic EHR experience is a plus, and a comprehensive benefits package is provided.

Qualifications

• Active, unrestricted Massachusetts NP or PA license (or ability to obtain prior to start)
• NP: MSN and national certification (ANCC or AANP) or PA: Master's degree and NCCPA certification
• Active DEA and MCSR (or ability to obtain prior to start)
• Current BLS certification
• Minimum of three (3) years of experience independently managing a full primary care patient panel
• Experience in ambulatory primary care, care coordination, and chronic disease management
• Preferred: value-based/population health or coordinated care model; Epic EHR experience

Responsibilities

• Independently manage a defined panel of primary care patients, providing longitudinal, relationship-based care
• Perform comprehensive assessments, diagnoses, and treatment of patients in an ambulatory primary care setting
• Manage acute and chronic conditions and supervise patient care plans
• Make clinically appropriate determinations regarding in-office management vs. referral
• Manage inbox, test results, patient messaging, and referrals
• Coordinate care with RNs, pharmacists, population health, behavioral health, and specialists
• Partner with patients and families for shared decision-making and whole-person care
• Provide consultation regarding hospitalized patients within the panel
• Support practice efficiency, quality initiatives, and standardized clinical workflows
