Qualifications

Responsibilities

• Coordinate comprehensive, longitudinal care for a patient panel.
• Perform exams, diagnoses, and treatments in primary care.
• Oversee care plans with referrals and communications to specialists as needed.
• Collaborate with patients and families to manage care plans.
