Job summary

Atrius Health, part of the Optum family, is seeking a Primary Care Provider - Advanced Practice Clinician (NP or PA) to join our Weymouth, MA team. You will deliver longitudinal primary care, manage chronic conditions, and collaborate with RNs and specialists in a value-based care model.

The role requires NP/PA certification, MA license, DEA, MCSR, and BLS with at least 3 years of ambulatory experience. Epic EHR experience is preferred, with competitive compensation and benefits offered.

Qualifications

• NP or PA with national cert (ANCC/AANP or NCCPA)
• Active MA NP/PA license (or ability to obtain)
• Active DEA license and MCSR (or ability to obtain)
• Current BLS certification

Responsibilities

• Independently manage a defined primary care panel with longitudinal, relationship-based care
• Perform comprehensive assessments, diagnoses, and treatment in ambulatory primary care
• Manage acute and chronic conditions and oversee care plans
• Determine in-office vs. specialty or emergency referrals
• Handle inbox duties, test results, patient messaging, and referrals
• Coordinate care with RNs, pharmacists, and other specialists
• Partner with patients and families for shared decision-making
• Provide consultation regarding hospitalized patients within the panel
• Support practice efficiency and standardized clinical workflows
