Qualifications

• APC with independent primary care experience in ambulatory settings.
• Experience managing a defined panel of primary care patients.
• Strong focus on longitudinal, population-based care and coordination.

Responsibilities

• Independently manage a defined panel of primary care patients, providing longitudinal, relationship-based care
• Perform comprehensive assessments, diagnoses, and treatment in ambulatory primary care
• Manage acute and chronic conditions and develop patient care plans
• Determine in-office management vs. referral to specialty or emergency care
• Handle inbox responsibilities, test results, patient messaging, medications, and referrals
• Coordinate care with RNs, pharmacists, population health, behavioral health, and specialists
• Partner with patients and families for shared decision-making and whole-person care
• Provide consultation on hospitalized patients within the assigned panel
• Support practice efficiency, quality initiatives, and standardized clinical workflows
