Job summary

Atrius Health, part of the Optum family, seeks an Advanced Practice Clinician (NP or PA) to join our Boston, MA team. You will provide comprehensive, longitudinal care for a defined patient panel in a primary care setting, including exam, diagnoses, and care coordination.

You will collaborate with patients, families and staff to manage care plans, monitor acute and chronic illnesses, and support hospital consultations, with a focus on high-quality patient outcomes and efficient care delivery.

Qualifications

• Graduate of an Accredited Nurse Practitioner or Physician Assistant program.
• Active, unrestricted Massachusetts NP or PA license.
• National Board Certification.
• Mass DPH Controlled Substances Registration active or ability to obtain post-hire.
• DEA licensure/prescriptive authority active or ability to obtain post-hire.
• Maintains BLS certification.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of a defined patient panel.
• Examination, diagnosis and treatment in a primary care setting with care plan supervision.
• Monitor and manage patients with acute and chronic illnesses; adjust regimens as needed.
• Collaborate with patients and families to manage care plans.
• Provide consultation for hospitalized patients in the panel and support staff efficiency.
