Job summary

CoxHealth is seeking an Ambulatory Care Coordinator I to support care teams within Population Health Services. You will coordinate efforts for specialty and managed care populations, communicate with patients about gaps in care, schedule appointments, and organize resources to improve access and outcomes.

Qualifications

• Requires Bachelor's Degree in Health/Human Services or 8 years related experience.
• 1 year of customer service experience required; clinic operations experience preferred.

Responsibilities

• Coordinate care for specialty and managed care populations under indirect supervision.
• Communicate with patients regarding gaps in care (medication adherence, screenings) and schedule appointments.
• Procure and organize resources (medications, financial assistance, ambulatory formulary) and assess social determinants of health.
• Must be team-oriented with strong leadership, communication, problem-solving, and organizational skills.
