CoxHealth is seeking an Ambulatory Care Coordinator I to support Population Health Services by coordinating care efforts for specialty and managed care populations. The role includes direct patient communications about gaps in care, scheduling, and resource coordination in clinical and community settings.

Ideal candidates will have a Bachelor’s degree or equivalent experience, 1 year in customer service, and strong EMR and Excel skills.

Qualifications

• Education: Bachelor’s Degree in Health/Human Services or equivalent; 8+ years related experience.
• Experience: At least 1 year in customer service.
• Skills: Proficient in EMR systems, Excel, and payer platforms; strong communication and organizational abilities.

Responsabilités

• Coordinate care activities within Population Health Services and related teams.
• Communicate with patients about open gaps in care (medication adherence, screenings).
• Schedule appointments and manage resources (medications, financial assistance, formulary).
• Assess social determinants of health to support patient outcomes.
• Work under supervision in clinical and community settings; maintain professional interactions.

Position works under indirect supervision and works in both clinical and community-based settings. Responsibilities can include, but are not limited to, communicating directly with patients regarding open gaps in care (e.g. medication adherence, breast cancer screenings, colorectal cancer screenings), scheduling appointments, procurement and organization of resources (e.g. medications, financial assistance, ambulatory formulary) and assessing social determinants of health.
