Job summary

AdvantageCare Physicians (ACPNY) is expanding its primary and specialty care practice across New York City and Long Island. This role provides professional nursing care and ensures adherence to patient satisfaction, safety, and quality standards established by ACPNY.

The position acts as the liaison among patients, staff and physicians, and supports the Advanced Care Model. It also includes organizing clinical supervision for the specialty care team and guiding scheduling and education.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor's Degree in Nursing preferred.
• Current unencumbered New York State Registered Nurse license.
• BLS.
• ACLS required for GI and Cardiology.

Responsibilities

• Provide professional nursing care to patients and support patient satisfaction, safety, and quality of care.
• Serve as primary liaison between patients, staff and physicians and support the Advanced Care Model.
• Organize and supervise the specialty patient care team, including delegation, mentoring, evaluating, scheduling and counseling.
