Job summary

UnitedHealth Group's UnitedHealthcare is seeking a Field Care Coordinator (Case Manager) to support a caseload of members with low-to-medium complexity medical/behavioral needs across Navajo and Apache Counties, including Show Low, AZ. This field-based role requires visiting members in their homes and long-term care settings to assess needs and coordinate care.

You will partner with teams, provide education, manage transitions after ER visits, and advocate for members, with a compensation

Qualifications

• 2+ years of case management experience serving members with Serious Mental Illness (SMI)
• 1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities
• 1+ years of experience with MS Office (Word, Excel, Outlook)
• Driver's License and access to reliable transportation; ability to travel within assigned territory ~50% of the time
• Ability to work in field visiting members in homes or facilities

Responsibilities

• Engage members face-to-face and/or telephonically to complete comprehensive needs assessments
• Develop and implement person-centered care plans addressing medical, behavioral, functional, cultural, and socioeconomic needs
• Partner and collaborate with internal care team, providers, and community resources to implement care plans
• Provide education and coaching to support member self-management of care needs and lifestyle changes
• Support proactive discharge planning and coordinate care transitions after ER, inpatient, or SNF admissions
• Advocate for members and families to ensure needs and choices are represented and supported by the health care team
