Job summary

UnitedHealthcare is seeking a Field Care Coordinator to manage a panel of members with low-to-medium complexity medical/behavioral needs across Maricopa County, AZ. You will assess, plan, educate and coordinate care in the field and via telecommunication to improve access and outcomes.

The role requires field visits to members in homes or long-term care facilities, strong collaboration with care teams, and a focus on social determinants of health. Travel is expected within the territory.

Qualifications

• 2+ years of case management experience serving members with SMI.
• 1+ years of case management experience serving elderly and/or disabled individuals.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel within territory.

Responsibilities

• Engage members in person or by phone to complete comprehensive needs assessments covering medical, behavioral, functional, cultural and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, social determinants of health, medication management and safety.
• Collaborate with internal care team, providers and community resources to implement care plans.
