Qualifications

• Driver’s license and reliable transportation for 50% field travel.

Responsibilities

• Engage members in person or by phone to complete needs assessments.
• Develop person-centered care plans addressing medical, behavioral, and social needs.
• Coordinate with care teams, providers, and community resources.
• Educate and coach members on self-management and lifestyle changes.
• Support discharge planning and care transitions after ER visits or SNF stays.
• Advocate for members to ensure their needs are represented by the team.
