Job summary

Eventus WholeHealth seeks a Clinician Support Specialist to assist clinicians and coordinate workflows across skilled nursing and assisted living facilities. You will support encounter documentation, chart preparation, and telehealth while enhancing patient communications and care quality.

This role requires strong organization, excellent customer service, and the ability to travel regularly to facilities. Certification as CMA/CNA/PCT is preferred, with at least one year of patient care

Qualifications

• High school diploma or equivalent required.
• Certified/Registered Medical Assistant, CNA, or PCT preferred.
• Minimum one year of patient care experience preferred; medical scribing experience preferred.
• Valid driver's license and reliable transportation.
• Strong customer service and communication skills.
• Strong organizational skills and ability to navigate multiple priorities.

Responsibilities

• Prepare for daily clinical rounds with patient information and schedules.
• Assist with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain patient schedules, admissions, discharges, and acute changes.
• Serve as liaison between clinicians, facility staff, patients, and families.
• Assist with triage needs and documentation.
• Organize and manage clinical documentation, including lab results and patient records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and route documentation.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and maintain patient census and records.
• Maintain professionalism within partner facilities.
