Eventus WholeHealth is hiring a Clinician Support Specialist to back our clinical care team in skilled nursing and assisted living facilities. You will coordinate workflows, assist with documentation, and support telehealth visits to ensure timely, high‑quality patient care.

You will work closely with Advanced Practice Clinicians and facility staff, managing schedules, records, and communications while traveling to facilities as needed. Strong organizational skills are essential.

Qualifications

• High school diploma or equivalent required.
• Certified/Registered Medical Assistant, CNA, or PCT preferred.
• Strong customer service and communication skills; ability to manage multiple tasks.

Responsibilities

• Prepare for daily clinical rounds with complete patient information and schedules.
• Assist with encounter documentation, data entry, and chart preparation.
• Coordinate patient schedules including admissions, discharges, and changes.
• Serve as liaison among clinicians, facility staff, patients and families to support care delivery.
• Assist with triage and documentation as directed by clinicians.
• Organize clinical documentation, including labs and diagnostic data.
• Host and support telehealth visits.
• Obtain and route patient consents and documentation.
• Participate in facility meetings and communicate updates to the team.
• Support discharge planning and maintain patient census and records.
