Qualifications

• Six months as Aging Care Manager 1 or County Caseworker 1.
• Completion of the County Social Casework Intern program.
• Six months of professional human services or nursing experience with a related bachelor’s degree.
• An equivalent combination of experience and training.

Responsibilities

• Case Management: Assess consumer needs, maintain records, and complete reassessments to ensure appropriate services
• Service Coordination: Arrange and coordinate services with community resources, hospitals, and agency programs
• Eligibility Review: Screen consumers for program eligibility and explain cost sharing, donation procedures, and appeals
• Information Gathering: Collect required documents and obtain written consent to access or share client information
• Client Support: Provide clear explanations about service options, associated costs, and available alternatives
• Intake Duties: Perform intake and referral responsibilities with close attention to accuracy and timely documentation
