Qualifications

• Six months as Aging Care Manager 1 or County Caseworker 1.
• Or completion of the County Social Casework Intern program.
• Six months of human services or nursing experience with a related bachelor’s degree.
• Equivalent combination of experience and training.

Responsibilities

• Assess consumer needs and maintain records for services.
• Coordinate services with community resources and hospitals.
• Explain program eligibility, cost sharing, and appeals.
• Collect required documents and obtain consent for data access.
• Explain service options and costs to clients.
• Perform intake and referrals with accurate documentation.
