Job summary

The Commonwealth of Pennsylvania’s Aging Service seeks an Aging Case Aide 2 in Wayne County to assist older adults with access to Medicare, LIHEAP, and community resources. You will conduct outreach, coordinate services, and support nutrition and volunteer activities to improve seniors’ well-being.

You will work full-time, typically 8:00 AM to 4:00 PM with travel to various sites. A background check is required and PA residency is expected.

Qualifications

• At least one year as Aging Case Aide 1.
• Two years of public contact work, including one year in a human services program.
• Equivalent combination of experience and training acceptable.

Responsibilities

• Benefits Guidance: Provide information and assistance on Medicare, PACE, LIHEAP, and related programs.
• Application Support: Help older adults complete forms and connect with needed community resources.
• Outreach Efforts: Conduct home visits and participate in countywide outreach activities.
• Program Coordination: Assist with senior center events, volunteer supervision, and participant assessments.
• Nutrition Services: Support meal distribution, sanitation procedures, and donation documentation.
• Record Keeping: Maintain accurate records and complete data entry in agency and statewide systems.
