Job summary

The Commonwealth of Pennsylvania is seeking an Aging Care Manager 3 in Honesdale, PA to join Wayne County AAA. You will assess seniors 60+ and coordinate protective services to promote safety, independence, and quality of life.

This full-time position requires strong judgment, compassionate service planning, and collaboration with health care providers, courts, and community partners. The on-site role may include evenings and occasional travel; an excellent benefits package is included.

Qualifications

• One year as an Aging Care Manager 2 or Aging Program Assessor; or
• One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

Responsibilities

• Conduct investigations into abuse, neglect, exploitation, or abandonment.
• Evaluate medical, social, psychological, financial, and environmental needs for older adults.
• Develop, implement, and oversee protective service plans that reduce or remove risk.
• Recommend court actions, coordinate interventions, and provide testimony when required.
• Work with service agencies, healthcare providers, courts, and community partners to support client needs.
• Provide direction and consultation to professional staff and review casework for quality and compliance.
