Qualifications

• Six months as an Aging Care Manager 1 or County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.

Responsibilities

• Needs Assessment: Evaluate social contacts, daily living activities, cognitive functioning, physical health, and available resources.
• Service Coordination: Connect older adults with agencies, hospitals, clinics, courts, and community organizations that support their needs.
• Comprehensive Reviews: Conduct detailed assessments of medical, social, psychological, and housing conditions to support effective care planning.
• Interagency Collaboration: Work with professional staff and community partners to discuss diagnoses and plans of treatment.
• Benefits Guidance: Assist older adults in managing income, accessing Social Security and Medicare, and understanding legislation that affects benefits.
• Case Documentation: Prepare social studies, case histories, reports, and records.

Education
