Job summary

Gentiva Health Services, Inc. is seeking a dedicated Afterhours RN Care Manager to ensure high-quality hospice care outside standard hours. You will provide clinical management for patients and families during on-call hours, including direct patient assessments and collaborations with the Interdisciplinary Group.

The ideal candidate will have a valid RN license, experience in acute care or hospice, and a strong understanding of hospice principles.

Qualifications

• Minimum one year of RN experience in healthcare.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person patient care during after-hours shifts.
• Assess and coordinate patient and family needs.
