Zusammenfassung

Assured Hospice, part of the LHC Group within the Optum family, is hiring an Afterhours Registered Nurse to deliver comprehensive hospice care after regular hours, including nights and weekends. The role requires timely evaluations, after-hours visits, and accurate documentation to support patient and family care.

Eligible candidates have an unrestricted RN license in the state, a valid driver’s license and reliable transportation.

Qualifikationen

• Current unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Aufgaben

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to after-hours crisis calls from patients and families.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct after-hours visits including admissions, death, recertification and routine visits.
• Identify and reassess patient/family needs at least every 15 days.
• Document problems, goals, interventions and patient/family response.
