Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.
• CPR certification preferred or able to obtain within 90 days of hire.
• 1+ years of clinical experience preferred.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond promptly and with urgency to after-hours crisis calls from patients and families.
• Conduct the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Conduct after-hours visits including admission, death, recertification, and routine visits.
• Identify and reassess patients/families’ physical, psychosocial, emotional, and environmental needs at least every 15 days.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
