Job summary

UnitedHealth Group is seeking an Afterhours Registered Nurse to deliver hospice care to patients and families after regular hours, including nights and weekends. You will triage needs by phone, perform initial nursing evaluations, and conduct afterhours visits in patients' homes.

Applicants must hold an unrestricted RN license and a valid driver's license or access to reliable transportation; CPR certification preferred and 1+ year of clinical experience.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license with access to reliable vehicle or public transit.

Responsibilities

• Triage patient needs over the phone until in-person nurse visit.
• Respond to afterhours crisis calls with urgent action.
• Perform initial nursing evaluation within 48 hours of referral.
• Conduct afterhours admissions, death, recertification, and routine visits.
• Assess patient/family psychosocial, physical, emotional, and environmental needs every 15 days.
• Document problems, goals, interventions, and patient/family responses.
