Job summary

UnitedHealth Group seeks an Afterhours Registered Nurse to provide comprehensive hospice care for patients and families after regular business hours, including nights and weekends. Primary responsibilities include phone triage, crisis response, and conducting in-person visits within 48 hours of referral.

The role requires current RN licensure in the state, a valid driver’s license, and a willingness to work afterhours in patients’ homes, with a competitive salary and benefits package.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Triage patient needs over the phone until a nurse can visit in person.
• Respond to crisis calls from patients and families after hours with urgency.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours admissions, death, recertification and routine visits as needed.
• Assess and reassess patient/family needs (physical, psychosocial, emotional, environmental) at least every 15 days.
