Job summary

UnitedHealth Group, a part of the Optum family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and families outside standard hours, including nights and weekends. You will triage by phone, perform initial nursing evaluations, and conduct afterhours visits.

Requirements include an unrestricted RN license in the state, a valid driver's license with reliable transportation, CPR preferred within 90 days, and 1+ years of clinical experience.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification preferred within 90 days of hire.
• 1+ years of clinical experience preferred.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to afterhours crisis calls with urgency.
• Conduct initial nursing evaluation within 48 hours of referral when assigned.
• Perform afterhours visits including admissions, death, recertification, and routine visits.
• Assess patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions, and patient/family response.
