Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond promptly to afterhours crisis calls from patients and families.
• Perform initial nursing evaluation within 48 hours of referral as part of a patient visit.
• Conduct afterhours visits including admissions, death visits, recertifications, and routine visits.
• Identify patient/family physical, psychosocial, emotional, and environmental needs and reassess at least every 15 days.
• Document problems, goals, interventions, and patient/family responses to hospice care.
