Job summary

UnitedHealth Group’s Heart of Hospice in Mississippi seeks an Afterhours Registered Nurse to provide comprehensive hospice care during nights and weekends. You will triage needs by phone, conduct in-person visits, and perform initial assessments to determine eligibility within 48 hours of referral.

The role requires an unrestricted RN license, a valid driver's license and reliable vehicle or public transport.

Qualifications

• Current unrestricted RN license in state of practice.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• MS-specific requirement: at least one year RN experience; LPN substitution allowed in MS.
• Current CPR certification or ability to obtain within 90 days.
• 1+ years of clinical experience preferred.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to crisis calls after-hours with urgency.
• Perform initial nursing evaluation within 48 hours of referral.
• Conduct afterhours visits including admissions, death visits, and recertifications.
• Assess patient/family needs every 15 days and adjust care plan as needed.
• Document problems, goals, interventions, and responses.
