Job summary

UnitedHealth Group is seeking an Afterhours Registered Nurse to provide comprehensive hospice care for patients and families outside regular hours, including nights and weekends. Primary duties include phone triage, crisis response, and performing nursing evaluations within 48 hours of referral.

Qualifications

• Current unrestricted RN licensure in the state of practice is required.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Triage patient needs over the phone until a nurse can visit in person.
• Respond promptly to crisis calls from patients and families after hours.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours visits including admission, death, recertification, and routine visits.
• Assess and reassess patient/family needs at least every 15 days.
• Document problems, goals, interventions and patient/family response to hospice care.
