Qualifications

Current unrestricted RN licensure in the state of practice.
Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
CPR certification preferred or ability to complete within 90 days of hire.

Responsibilities

Triage patient needs over the phone until an in-person visit by a nurse.
Respond to crisis calls after hours with urgency.
Make initial nursing evaluation within 48 hours of referral when assigned.
Perform afterhours visits including admission, death, recertification, and routine visits.
Identify and reassess patient/family needs every 15 days or as needed.
Document problems, goals, interventions, and patient/family response to hospice care.
