Job summary

UnitedHealth Group's Heart n Home Hospice, part of LHC Group, is seeking an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and families after regular hours, including nights and weekends.

You will triage needs by phone, perform afterhours admissions and visits, assess eligibility within 48 hours, document care, and support patients in diverse home environments while upholding our commitment to compassionate, high-quality care.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Triage patient needs over the phone until an in-person nurse visit.
• Respond to afterhours crisis calls with urgency.
• Conduct initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Perform afterhours visits including admissions, death visits, recertifications, and routine visits.
• Identify patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions, and patient/family responses to care.
