Qualifications

• Current RN license in the state of employment and an active RN license.
• Experience providing hospice or home health care and care coordination.
• Strong communication and documentation skills with EMR systems.

Responsibilities

• Provide direct in-person care during after-hours, weekends, and on-call shifts.
• Assess physical, emotional, psychosocial, and spiritual needs of patients and families.
• Coordinate with the Interdisciplinary Group (IDG) for care planning.
• Develop and revise the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression and symptom management.
• Maintain timely clinical documentation including plans and notes.
