Job summary

Gentiva Hospice is seeking an Afterhours RN Care Manager in Winfield, Kansas, to deliver high-quality hospice care outside standard hours. You will assess and coordinate care, perform routine visits, and maintain clinical oversight for assigned patients and families.

As an active IDG member, you will collaborate with physicians and team members, manage documentation, and support regulatory compliance while delivering compassionate end-of-life care.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person hospice care during after-hours shifts.
• Assess and coordinate patient and family needs after hours.
• Maintain primary clinical oversight for assigned patients.
• Educate patients and families on disease progression and symptom management.
• Communicate changes in patient condition to physicians and IDG.
