Job summary

Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. You’ll provide direct patient care in homes or facilities, coordinate with the daytime team, and ensure timely symptom management and documentation.

The role emphasizes compassionate, autonomous nursing with CPR certification, an active RN license, and 1+ year of nursing experience.

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain)
• 1+ years of nursing experience
• A hospice heart: compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and the ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes, facilities, or wherever they reside
• Valid driver’s license, automobile insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays as the primary liaison between patients, families, physicians, and the hospice team
• Provide direct patient care and support after normal business hours
• Document clinical visits and communications accurately
• Participate in Quality Assessment and Performance Improvement programs
• Work independently while coordinating with the daytime care team
• Assist with education of patients, families, or caregivers as outlined in the Plan of Care
• Maintain records and ensure compliance with policies and regulatory requirements
