Job summary

VIA Health Partners in North Carolina is seeking an experienced registered nurse to coordinate hospice and palliative care for patients and families in the community.

You will conduct nursing assessments, visit patients at home, follow and adjust care plans, and collaborate with physicians, social workers, and other care team members to ensure compassionate, high-quality care. CPR certification and pediatric experience are valued; CHPN is preferred.

Qualifications

• Diploma or Associate's degree in nursing required; Bachelor’s degree preferred.
• Current RN license in good standing (NLC/Compact multi-state license).
• CPR certification required; CHPN preferred.
• Minimum 2 years of related hospice/palliative care experience.

Responsibilities

• Coordinate patient and caregiver care across the plan of care.
• Perform nursing assessments and make home visits.
• Follow and update plans of care; provide education.
• Emergency triage by phone and home visits.
• Maintain clinical documentation and records.
• Collaborate with interdisciplinary team to coordinate care.
