Job summary

VIA Health Partners is a community-based, not-for-profit hospice and palliative care provider in North Carolina. We seek a registered nurse to coordinate patient and caregiver care, conduct nursing assessments, update plans of care, and educate families to support independence and comfort.

Requirements include an active RN license, CPR certification, and typically 1–2 years in inpatient or hospice/palliative settings.

Qualifications

• Diploma or associate degree in nursing required; Bachelor’s Degree preferred.
• Current, active RN license in good standing.
• CPR certification required; CHPN preferred.
• Minimum 1–2 years in inpatient units; hospice/palliative care experience preferred.
• Computer proficiency required.

Responsibilities

• Assumes responsibility for coordination and delivery of patient/caregiver care.
• Conducts nursing assessments to manage patient/caregiver needs.
• Develops, implements, evaluates, and updates the plan of care to ensure uninterrupted care.
• Facilitates patient and caregiver independence where possible.
• Provides education to patients and families; communicates with IDG and other health professionals.
