Job summary

VIA Health Partners in Fort Mill, SC is seeking a skilled Registered Nurse to coordinate patient and caregiver care in end-of-life settings. You will perform nursing assessments, visit patients, implement and revise plans of care, and provide education to families.

This role emphasizes multidisciplinary collaboration, emergency triage by phone and visits, and adherence to VIA policies, while supporting the community with compassionate, patient-centered hospice and palliative care.

Qualifications

• Diploma or Associates Degree in nursing required; Bachelor’s Degree preferred.
• Current RN license (compact multi-state) required.
• CPR certification required; CHPN preferred.
• Minimum 2 years related hospice/palliative care experience; pediatric exposure may be required for Kids Path.

Responsibilities

• Coordinate patient/caregiver care across settings.
• Conduct nursing assessments and implement plans of care.
• Perform visits and emergency triage by phone and in person.
• Educate patients and families; document care and responses.
• Maintain professional licensure and adhere to policies.
